Step 1 : Click on the patient name on the patient toolbar, and in the side menu that appears, click on the option follow up. 

Step 2 : Choose the provider you want the follow up to be done by, select the date the follow up has to be done for and put in any comments for the follow up to be done by the provider. Then click on save.
