How to Set Up and Configure Exam - History Forms

Editing Forms in VETport
Go to the hamburger in upper left hand corner qne dlidk on EMR Management. Here you will see a list of forms and templates. Click on history and exam form. Here you can
            1. Add a new form 
            2 Drag and drop in order you want to see them. 
              (note the 1st on on top is the default form.)
            3. Make active or inactive.
            4. Manage the form.
            5. Edit the species and form name.
            6. Copy the form. This is helpful if you are using a form for different

Edit a line item:
1. Add a component line to the form.
2. Where the line will be added, at bottom of page. 
3. Put a check here if you want the line to be mandatory. 
4. If checked a comment box will be added on the bottom of the box for doctor's notes. 
5. Delete the line.
6. Drag and Drop the line in what order you want to see them in 
7. Pick the type of line it will be: Text Box, Paragraph, Drop Down, Radio Button, Checkbox, Attachment.
                                                              


                                                                                                                                    

After clicking on # 7 above the screen below will appear. 
1. You can add a report card to a line and each time you use the form it will attach the report card. 
2. You can add an attachment to a line and each time you use the form it will attach the report card. 
3. If you choose a type that requires a list, type it here and hit return. 
4a. Add a report card to an option you added above. It will only be displayed if the check mark is performed on the form.

5. Mark the option as a normal finding 
6. Allow the option to be edited. 
7. Make this line mandatory.
